I will try and sort these by author/member who initially posted the picture, hoping that gives us the highest chance of re-finding the offline pictures. This is mostly useful for the upcoming picture request thread when the main part of the project is finished.
If possible, the original post will be linked at the bottom of every individual accident's information.

Filming this crash on my video camera I feared the worst.
Saw a body hanging out of the door window as it rolled a few times and a crash helmet come flying off.
Thankfully the crew sustained only minor injuries.

Nicholas Morley and co-driver Matthew McConville apparently panicked and tried to do a runner in another car, after they hit a car with an elderly married couple - they both died.

The rally was apparently legal, but subject to speed limits. Which were probably broken in this case. The 2007 Gumball 3000 was cancelled after the accident, and I doubt it will take place next year, if ever again.

The extremely fast roads of the Toolangi Forest along with the pressure of being the last event of the ARC took its toll on some competitors at Rally Melbourne on October 2-3, 2004.

The most significant was the accident of Darren Windus and Michael Cains, who hit a tree at high speed after the flying finish on Special Stage 9.

Darren was trapped in the vehicle for some time as the roof was bent down over the driver's side, while Michael was able to be helped from the car.

Both were air lifted by helicopter to the Alfred Trauma Centre in Melbourne.

In spite of the massive damage to the car, it's quite clear that without the roll cage and safety equipment such as seat belts, the situation would have been far worse.

Article is dedicated to Åkesson's accident, but there's some things that make me doubt that crash is fatal:
1. Caption says that it's place of Mårtensson/Åkesson accident, but not mentioned that car belongs to them;
2. Car doesn't seems to be Volvo 240; for me it seems to be WRC-based rally car;
3. According to article, car crashed with the roof just behind Åkesson's seat; in photo roof looks undamaged while driver's side (if car has left-hand drive like Volvo 240) seems to receive impact.

Accident happened in early 1980s. I'm not sure if I translated comments well, but according to Arunas Petrovas, driver was decapitated and co-driver suffered severe injuries. Accident happened because at same time several farming vehicles including harvester combines were at road. Crew was presumably from Marijampole and driver's surname probably was Matulevičius.
Photo belongs to different accident: Eugenijus Kurtinaitis and K.Matulevičius in mid-1980s, unknown rally, both crew members survived:

In the bottom pic (= THE OFFLINE ONE) there is also something resembling a crash at AVUS, but I'm not sure about any of that. The caption to the 2nd III. Well, I cannot seem to disprove this, no matter how hard I try. Given the fact that there was a total of (3) Porsche 904 GTSes entered, it could be anyone of (6) drivers, but Jesper's ID seems to be the most probable.

I scanned the picture below, from the July 1964 issue of Car and Driver Magazine. You can clearly see the driver. Maybe one of our "Helmet Quizzers" will recognize this fella?

This is getting increasingly harder as we're working on posts that are >15 years old. It's not the amount of posts that need to be added to the indexing but the (often) lack of information on some of the photos. About 60% of the Rally Accidents thread's content (page 42 - 97) is indexed. I'll see what's waiting for me on the pages to come and make a decision on how/if I will proceed, then.

The Indexing can be considered done.

I have decided to not include the first 11 pages of the Rally Accidents thread.
The reason behind that is that on page 11, there was only one entry where the pictures were still available, which is this one: viewtopic.php?p=12988#p12988. This is the last entry (and the only one from page 11) that made it into the indexing.

-Replace the links in the indexing

Since I want to update the CPDB adding the Italian circuits fatalities that were found recently (I haven't been updating the CPDB with them for 3-4 years...), I've just added some Italian rally fatal accidents.
You might want to check them out (so that we don't add them twice).
