On the 16th lap of the race, Ross Bentley went off the road and stalled his car in the Turn 3 chicane. Four members of the safety crew rushed over to assist in push-starting Bentley's Lola, which was able to re-fire. Upon restarting the car, the marshals quickly ran to the right side of the track, where their post was located. Willy T. Ribbs, unable to see the marshals beforehand due to the concrete walls that delineated the chicane, hit three of the four marshals: two of them were hit by Ribbs' rear wing, while one, Jean-Patrick Hein, was ran over by the left rear wheel.
Hein, 22, was given a tracheotomy at the scene and then rushed to St. Paul Hospital in Vancouver, where he died later that night. Bradley Weeks, who took the brunt of Ribbs' rear wing, survived with a broken leg.
